We are honored that you have chosen Nova Pediatrics for your child's care. Your comfort and convenience are our priority, and we strive to make every visit to our office a pleasant experience. To help you get acquainted with our office and first visit procedures, we have included helpful information on this page.
Patient Forms
Since we value your time, to expedite the new patient registration we ask that you arrive at least 15 minutes prior to your appointment to fill out new patient paperwork. You may also download and print these forms from our website. Print them out and either fax (615)-454-6336 them to our office prior to your visit or bring them at the time of your appointment to further reduce your wait time.
ALSO please bring all previous medical records, immunization cards, and insurance cards at the the time of the first appointment.
First Visit
New patient visits are scheduled for 20 minutes to allow you sufficient time to get to know our doctor and staff and have all your questions and concerns answered in detail.
